5. Updated Professional council certificate
6. Updated PIS
7. Recommendation letter of current office
8. Remote and rural service record
9. Service record, Leave and other records
10. Performance appraisal record of last two fiscal year
11. Staff adjustment and update if any
12. Application form with detail information (Name, No., Position, Office and contact information)
